FATHER OF THE YEAR NOMINATION FORM

It is the intent of FATHERS INCORPORATED (“FATHERS”) to recognize a nominee that changed or impacted the lives of his family and community for the better, in the year immediately prior to receiving the award as well as their past achievements and ongoing contributions to the concept of family and community. To be considered the nominee must be a father:

Who is a resident of Washington DC or its Surrounding Communities in Maryland and Northern Virginia.
Whose Challenges and triumphs are manifested in his life experience and personal journey, beyond their profession and career achievements (Any job-related task or duty for which a nominee is paid a salary will not be considered).
Whose accomplishments went beyond “normal” expectations and created a lasting and beneficial impact on his family and residents of his community.
Who took risks and achieved what was needed and whose leadership had a positive impact on a wide range of people’s lives.
Who resides in the community being considered; and
Who possesses excellent character and integrity and whose actions and achievements reflect the values and founding principles of FATHERS: BROTHERHOOD, MANHOOD, FATHERHOOD, and GENTLEMANSHIP, as defined.

Nomination Instructions:

Provide all information fully and completely. Incomplete nominations will not be considered.
Return completed nomination NO LATER THAN March 31, 2024, TO BE CONSIDERED FOR THAT YEAR’S AWARD.
A nominee will be considered once, and only the highest evaluated score for a nominee with multiple nominations will be considered. Multiple nominations for the same father will not guarantee that father the award and are not encouraged.
The winner will be featured on the FATHERS website following the award announcement and receive a cash award and/or a 7-Day vacation at Florida Spa and Resort. The winner may also be asked to sign FATHERS Father of the Year Award Agreement (optional).

Nomination Form

As part of the Selection and Award Process, Selected Nominees may be required to participate in a personal interview. If selected for the award, Nominees will be notified by email of the interview date and time. The interview will be conducted virtually, via ZOOM or another compatible format. The Nominees must be available for the interview as this is a requirement of the Father of the Year Award – 2023 Program eligibility.
